Fusion21 Foundation supports Healthy@Home rollout for Liverpool social housing residents

A pioneering project supported by the Fusion21 Foundation has moved from pilot stage to rollout, helping social housing residents in Liverpool access more joined-up housing, health and wellbeing support.

Healthy@Home, delivered by Prima Group in partnership with Mersey Care NHS Foundation Trust, builds on an initiative supported by the Fusion21 Foundation in 2024. The project explored how housing and health information could be used together to better understand residents' needs, enable earlier intervention and help tackle health inequalities.

From pilot project to resident rollout

Following the pilot phase, Healthy@Home is now being rolled out to Prima Group residents in Liverpool.

More than 220 tenants across the Wavertree and Picton areas have been invited to take part in the programme, which brings together housing professionals and Integrated Care Teams to identify practical ways of improving residents’ health, wellbeing, safety and independence.

Examples of support may include:

  • Identifying housing improvements that could benefit residents living with respiratory conditions
  • Recommending adaptations that help people remain independent in their homes
  • Connecting residents with appropriate health and wellbeing support services
  • Addressing issues earlier before they escalate into more significant challenges

By working together, housing and healthcare professionals can take a more preventative approach to supporting residents.

Tackling health inequalities through partnership

Social housing providers are uniquely placed to support communities experiencing some of the most significant health inequalities.

By strengthening collaboration between housing and healthcare services, Healthy@Home aims to improve quality of life, help residents remain independent for longer and target support more effectively.

Creating a model for wider change

One of the long-term ambitions of Healthy@Home is to develop a framework for ongoing collaboration between housing associations and NHS organisations that can be scaled more widely across the UK.
